All forms are in Adobe PDF format unless noted otherwise. Volunteer Application: All Adults working with Teams (Team Managers, Co-Team Managers, Volunteer Parents) must be an approved Volunteer. AISD guidelines require two (2) approved volunteers at all meetings. Team Managers should encourage every parent to complete an application. If your team meets at someone's house, every adult and child over the age of 10 at that house must be an approved volunteer or student volunteer. Complete the Volunteer Application and submit it to the Community Programs Department, Susan Caldwell, at the A.I.S.D. Administration Building located at 1333 W. Pioneer Parkway, Arlington, TX 76013, (817) 460-4611. Make sure that you list Destination Imagination in the blank for the booster club you are working with. You can complete a Volunteer Application On Line at AISD Volunteers website. If this is your first time Regular Application. If you are a returning volunteer select Returning Volunteer Application. Make sure you include Destination Imagination in one of the location blocks on the online form. Student Volunteer Application: Students should not use the online application when making application. A parent signature is required on all student applications. Click here for the AISD Student Volunteer Application. Meeting Location Form: Every team must complete a Meeting Location Form. If your team is meeting at someone's home every adult and child over the age of 10 in that home must been an approved volunteer or student volunteer. Drivers Form: Any one driving the team to functions must be an approved driver. Your school will pay for 2 drivers checks. Complete the Drivers Form and submit with the appropriate documentation to the address on the top of the form. Consult with your school coordinator on how to pay for additional drivers checks. Permission to participate in Extracurricular Activity: Check in your schools office to obtain a copy of this form. This form must be completed for each Arlington ISD student participating in Destination Imagination. (This form not online) Fund Raising If your team plans to do on-campus fund raising you will need to complete some additional paperwork.
