Forms and Documents for Teams and Team Managers

Tournament Data Forms and Expense Reports (these should be available at www.idodi.org by January)

Regional Tournament Registration:   These forms must be turned in to your school or district coordinator  by December 3, 2007.   Also note that every school district handles payment of tournament registration fees differently.  If you have questions, contact your coordinator.    Regional Tournament Registration form (MSWord)  (PDF)      Download a MS Word Fill in Form Version of the registration form.

Mid Cities Medical Release - The team manager must have a Medical Release for every team member participating in the Regional Tournament.  DO NOT turn these in with your Tournament Registration.  Bring them to the tournament.  We will check to make sure that you have them before you can check in at the tournament.

Mid Cities Media Release - Parents must sign a media release for team members participating in the Regional Tournament.  DO NOT turn this form in with your tournament registration.  Bring them to the tournament.  You will need these forms when you check in at the tournament.

Tournament Invitation - A Microsoft word document you can print and give to parents, grandparents, teachers and friends inviting them to the tournament.  Download:  (MSWord)  (PDF)

TEAM CHEER ADS:  Cheer on your team, team manager, school, or district by placing an ad in the Tournament Program.  Click the link for more information.

T-Shirts  Click link at left to see preliminary design of T-Shirts.  T-Shirt orders are due by December 3, 2007.   Check with your school or school district coordinator if you have questions.  T-Shirt Order (PDF).   Only a limited number of T-Shirts will be available at the tournament.

Regional Pins:  Click link at left to see preliminary design of the Regional Pins.   You may pre-order pins at the same time that you order T-Shirts.  Pins may also be purchased at the Regional Tournament.   Pin orders are due to your school or school district coordinator by December 3, 2007.     Pin Order Form.   (PDF)
